Responsibilities

• Promote high quality standards of care through: Leadership and supervision of the Care Team, Person Centred Approach, Evidence based practice, Effective communication, Accurate, factual and timely documentation of clinical governance and adhere to Policies and Procedures of the home
• Liase effectively with other professionals and regulatory bodies through: Referrals to other professionals as required, Attend M.D.T meetings and ensure actions are completed, Contributing to D.S.T process for funding, Being responsible and supportive of other professionals involvement, Complete regulatory CQC notifications
• Ensure safety and well-being of residents through: Completing thorough pre-admission assessments, Care planning according to individual needs, Risk assessment and management, Review and evaluation of resident’s care, Addressing environmental factors that may impact on safety and comfort of residents, Application of safeguarding process when required, Addressing and responding to complaints, Ordering, administering and management of medication storage, ensuring systems of monitoring completed, Application of Mental Capacity Act 2005, contributing to Best Interest decisions, and DOL’s referral
